WIC MEMBERSHIP APPLICATION

To apply for a WIC Membership, follow these simple instructions:

1. Open the .pdf Membership Application.
   (Adobe Acrobat reader required. Click here to download the free reader.)

2. Click inside of the name field and type in your name. Use your mouse or the tab key on your keyboard to move to the next field. Fill in all required information.

3. After completing the form, click the Print button on the Acrobat toolbar.

4. Sign the signature area below the credit card information.

5. Fax the form to 702-386-8042, or if sending payment by check, mail the form and your check made payable to Las Vegas Women in Communications to:
