WIC MEMBERSHIP CHANGE FORM

To make a change in your membership information, follow these simple instructions:

1. Open the .pdf Membership Change Form.
   (Adobe Acrobat reader required. Click here to download the free reader.)

2. Click inside of the first field and type in the effective date of the change. Use your mouse or the tab key on your keyboard to move to the next field. Fill in all required information.

3. After completing the form, click the Print button on the Acrobat toolbar.

4. Fax the form to 702-386-8042, or mail the form to:
